Cancerkompisar Network at Large Cancerkompisar are here to help "No one should get sick just by being a relative" Cancerkompisar helps you, a relative of someone who is affected by cancer. We know how tough the life gets when someone you love is seriously ill. The fear, powerlessness and frustration. In the meantime, the life should go on as usual. You need to pick up the kids from school, do the laundry, celebrate birthdays and do your usual routines. All of this with a thousand thoughts and questions on your mind. How will it go? What happens now? What happens then? You should not be going through this alone. Cancerkompisar can help you find a cancer buddy - someone who is experiencing or have experienced the same thing as you do now. You and your cancer buddy can give each other advice and provide support in a situation that no one has to cope with alone. You meet and talk, or keep in touch by phone and e-mail. We also have an online forum where hundreds of families share their experiences and help each other. Cancerkompisar is a non-profit organization. We are funded by private donations and grants from state and county. It doesn’t cost anything, both to be and to have a cancer buddy. Our vision is that no one should get sick just by being a relative. Not you. Not anyone else. Visit Site Medicon Village AB Cancerkomisar

Amistad Network at Large Amistad came to life in 1982, begun by families of adults with severe and persistent mental illness (SPMI). These families knew that what their loved ones really needed was something not provided by other agencies -- a safe place to spend their abundant free time where they would be treated respectfully and where they could engage in useful activities. Amistad has grown over the years, however we remain committed to the original vision. We are a community where everyone is always welcome on a first name basis without regard to diagnosis and where everyone is treated with dignity and respect. We do the little things -- remember birthdays, celebrate accomplishments, take pictures, and memorialize members when they pass. We became a recognized 501(c)(3) corporation in 1996. Originally known as a social club, in 2001 the members voted to describe themselves as a Peer Support and Recovery Center. This name change reflects the awareness that Amistad has grown into a community of people committed to health, mutual support and recovery. In the past few years Amistad has become a leader in developing services that are organized and delivered by peers. We now operate three individual programs -- the original Peer Support and Recovery Center in Portland, we have expanded to include a Peer Support program in the Emergency Room at Maine Medical Center, and established a Peer Support Program inside Riverview Psychiatric Center. Banyan Mental Health Network at Large Banyan Treatment Center stands as a leader in providing growth through recovery by offering residential mental health care for all individuals. We understand the hardships many of our patients and families go through when a loved one is struggling with a mood or thought disorder. Our Mental Health Program is designed to treat a wide spectrum of mental health disorders and co-occurring disorders, at the inpatient residential level of care. Our Mental Health Program in Boca offers around the clock medical care for those with mental health disorders in a non-hospital setting. In order to attend Banyan Boca Raton's Residential Mental Health Program, the patient’s primary diagnosis must be mental illness. All of our treatment programs allow our patients to grow through recovery and we provide them with the necessary tools for future success and happiness. Many patients suffer from both mental health disorders and substance abuse disorders. When this occurs, the diagnoses are combined and referred to as “co-occurring” or “dual-diagnosis.” Many of our patients at our mental health program in Boca fall under this category, and are considered dual-diagnosis. We take great pride in being able to offer compassionate treatment for these conditions individually or combined. Our treatment programs include residential treatment and aftercare. We offer one staff member for every four patients, bringing truly personalized care. Residential treatment lasts from 7-30 days, depending on patient needs. We also offer medical care to address any challenges our patients may experience in their recovery. Contact our Banyan Mental Health professionals today to learn how we can help you or someone you love. Visit Site 1000 NW 15th St

VOCAL - Virginia Organization of Consumers Asserting Leadership Network at Large We are a statewide community, support network, social change movement and self-help education program. Our organization is created by and for people who have experienced emotional turbulence, mental health crisis, or extreme states of consciousness (commonly labeled as "mental illness”). VOCAL focuses on creating broad-scale social change, as well as change in the lives of individuals. We work to transform the mental health system -- and create alternatives to the system -- by promoting mental health recovery, self-determination and peer leadership. As people who have personally experienced mental health crisis, we work to create programs that respect the inherent worth and dignity of all people, regardless of their current or past mental state, diagnosis, or use of medications. We value the worth and dignity of each individual, the right to self-determination, and the important contributions of peer support and self-help. Visit Site 1904 Byrd Avenue, Suite 111

Oregon Research Institute Network at Large Our Mediterranean Lifestyle Program (MLP) is a comprehensive lifestyle intervention to reduce CHD risk in women with t2 diabetes. The MLP consists of the Mediterranean diet, PA, support & stress-mgt. training. Social isolation appears to be an independent risk factor for CHD in women and the impact of depression on heart attack survival has been shown to be mediated by perceived social support. The program included group support delivered by trained lay leaders to facilitate support groups. Visit Site 1715 Franklin Blvd

Campesinos Sin Fronteras Network at Large Campesinos Sin Fronteras has been in existence since 1997 and officially became a non-profit 501c3 organization in 1999. CSF’s client population consists primarily of migrant and seasonal farmworkers and their families, but includes many other low-income Hispanics who live along the US-Mexico border in Yuma County, Arizona. CSF provides services to a minimum of 10,000 people on an annual basis. Campesinos Sin Fronteras was founded by a group of farmworker and ex-farmworker community leaders who were concerned about the lack of health and social services and advocacy for the farmworker population. The idea behind the organization was to create a community-wide partnership between farmworkers and community leaders representing local organizations, local government, churches, and civic groups. CSF has since evolved into an organization with multiple health and housing programs that utilizes a grassroots Promotora Model to reach its clients in a linguistically and culturally appropriate way. The organization currently serves Yuma and the rural Arizona communities of Somerton, Gadsden, San Luis, Wellton, Dateland, and surrounding Colonias. CSF seeks to empower our community by promoting health, facilitating access to health services, and helping our clients to attain safe and affordable housing. CSF’s youth services focus on substance abuse prevention, leadership skills, and preparing young people for academic success. Visit Site P.O. Box 423

Penumbra Network at Large Penumbra began in 1985, by providing the first supported accommodation services in Scotland, enabling people with long term mental health issues to live with independence, supported by pioneering community based services. We started then as we meant to go on – by providing innovative solutions to gaps in mental health provisions based on the rights of service users to choice, dignity and the expectation of recovery. Since then our growth has encompassed a range of new individual and group projects, as well as negotiating and tendering for work from statutory agencies. Our innovative drive has continued from our first ever service. We are a well-established, financially secure, quality driven organisation with a commitment to ensuring fulfilling, contributing and satisfying lives for the people we support. Over the past 10 years we have also been providing services to people with a dual diagnosis, particularly people with addictions, and people who are homeless or at risk of homelessness. We currently employ 380 FTE employees and our turnover will be approximately £10m in this financial year. Visit Site Norton Park

S.H.A.R.E. Sharing Hope, Acceptance, Resilience and Experience. A burns peer support program - NSW Agency for Clinical Innovation Network at Large S.H.A.R.E. (Sharing Hope, Acceptance, Resilience and Experience), is a formalised peer support program for patients with a burn injury who are inpatients or outpatients at one of three New South Wales (NSW) Health sites. The program is an initiative of, and coordinated by, the NSW Agency for Clinical Innovation (ACI). Burn survivors are assessed via a formalised recruitment, screening and training process before being deemed ‘emotionally ready’ to undertake the role of peer supporter. The burns patient is then peer matched with a burns survivor based on certain selection characteristics. S.H.A.R.E. is championed by volunteers who are burn survivors, who share their story in an endeavour to improve patient outcomes. The engagement of burn survivors as Peer Support Volunteers offers patients a support system that is based on the fundamental premise of ‘shared experience’, something beyond what can be offered by the health professional. Whilst the volunteer and patient share the commonality of shared trauma, the group is otherwise diverse, hence the process of empowering patients in the selection of volunteers in nominating preferred characteristics such as age, gender, culture, and injury site. The Phoenix Recovery Support Services Network at Large The Phoenix Recovery Support Services is an Ohio Department of Mental Health and Addiction Services Certified Peer Run Organization, a Preferred Recovery Residence of the Stark County Mental Health and Addiction Recovery Service Board, a Certified Associate of Ohio Recovery Housing, and our Recovery Coaches are Certified Peer Recovery Supporters by the Ohio Department of Mental Health and Addiction Services? • We were the largest Community Based Stakeholder in the 2013 Recovery Housing in the State of Ohio: Findings and Recommendations from an Environmental Scan - conducted by the Center for Social Innovations and the Ohio Council of Behavioral Health and Family Services Providers • Our Founder was a core member of the BRSS TACS team he was chosen to represent Persons in Recovery for the State; this team put together the definition of Recovery for Ohio and developed the Integrated Peer Supporter Training Model that OhMHAS has recently Certified along with its Facilitator Training. • We have been an Approved Service Provider through three rounds (12years) of the Presidential Initiative Access to Recovery: Services included ~Peer to Peer Mentoring, ~Recovery Coaching, ~Daily Life Skills, ~Drug Free Transitional Housing • We were Stark Counties first Certified Associate of Ohio Recovery Housing. We operate 2-level1, 3-level2, and 1-level3 Recovery Housing Programs. • After the ACA kicked in and Mercy Medical Center kicked all the 12 Step Meetings out of their building which consisted of well over 100 meetings a year for the local Recovery Community along with the old Canton Arid Club turning a rye at the same time abandoning the meeting hall; we stepped up with the opportunity to take over and purchase that building to help preserve the Recovery Community and provide cost effective meeting hall spaces. • Five years ago We felt it was our duty to serve the Men who Served Our Great Nation by protecting and securing the Freedoms that even though once we threw away in the grasps of active addiction today Recovery has made possible for us. So we adapted our Rehabilitation Care Institute into an IOP Residence that serves VA connected Military Veterans. • We are a self sustaining, Recovery Focused, Person Centered, Person Driven, Life Saving, group of Recovering Advocates that have been humbly grinding out and giving back in the trenches of this #AddictionPandemic since our inception in 2005. The innovation is in our promotion from within which occurs when an individual rises from the ashes of his own destructions and is able to harness the power of his story to share his hope with the next man that he too shall Recover. Amistad Inc Network at Large Recovery. Relationships. Respect. Amistad came to life in 1982, begun by families of adults with severe and persistent mental illness (SPMI). These families knew that what their loved ones really needed was something not provided by other agencies -- a safe place to spend their abundant free time where they would be treated respectfully and where they could engage in useful activities. Friend for Life Cancer Support Network Network at Large Established in 1988, Friend for Life is a network of cancer survivors and co-survivors (caregivers) who serve as trained volunteers, providing one-on-one emotional and psychosocial support to persons recently diagnosed with cancer, and their loved ones. Facing cancer with someone who has been through a similar experience helps restore a sense of stability during a tumultuous time and to navigate the path through diagnosis, treatment and recovery. Support is provided by cancer survivors or caregivers who have received training by healthcare professionals to prepare them for their role as peer mentors. Support is available for persons diagnosed with cancer for the first time, for those with recurrence and metastasis. CASTLE Network at Large Hello, I am a Public Health Nurse working in a multi-component project called CASTLE. CASTLE is Creating Access to Cancer Screening in the Living Environment. CASTLE works with identified populations who experience life challenges to educate, promote, address barriers to increase access and community capacity for cancer screening tests through network partners, community health brokers and social media. This project was funded by the Public Health Agency of Canada and implemented in 6 regions. The CASTLE toolkit is unique in that it provides users with information about the process in developing the project for their replication. It also includes a comprehensive training curriculum for Community Health Brokers as well as resources that may be used freely and widely. We believe that CDC would be an excellent portal to disseminate the toolkit. http://castlenow.ca/en/toolkit_documents Visit Site 1447 Upper Ottawa St. Unitl 11
